Psychiatry

Please note that our psychiatry services are only provided within the province of Ontario. Services are provided to patients all over Ontario through a virtual platform, as long as they have a valid OHIP card.

Please note that for our psychiatry services, we do NOT accept any direct appointments from patients. To schedule sessions with our psychiatrist, Dr. Nimrit Bath, please have your primary care provider (family doctor or nurse practitioner) send a referral for Dr. Nimrit Bath via fax to: 343-451-6461. If there are issues with fax, please have your primary care provider (family doctor or nurse practitioner) send the referral via e-mail to hello@cciph.ca

Please note: We currently only provide psychiatric services for patients that are 18+.

Please note: Due to constrictions of time and to best utilize services Dr. Nimrit Bath will not be providing new ASD or ADHD diagnostic evaluations. These are best achieved through psychological testing and multidisciplinary care.

Please note: For our psychiatry services, we do not complete WSIB assessments as our psychiatrist is not a WSIB provider. The psychiatrist provides assessment and treatment for OHIP-covered services and is not a substitute for a disability assessment that would be through WSIB. In these cases, notes after our OHIP-covered assessment are sent to your primary care provider (family doctor or nurse practitioner) with recommendations for them to implement. Your primary care provider can answer any questions WSIB may have based on the consult notes sent to them as they are likely the ones assisting with this process. Thank you for your understanding.

Please ensure that your primary care provider includes all of the following components in the referral including:

  • Up-to-date patient contact information

  • Reason for referral

  • Previous psychiatric assessments and medication trials

  • Cognitive assessments

  • Investigations – bloodwork/ECG/head imaging

  • Patient’s valid OHIP card

  • Physician or Nurse Practitioner’s OHIP billing number

  • Any other pertinent information

Please note that if all of the required information outlined above is not received, it could lead to a delay or cancellation in your referral being processed.
